About Community Health

We work to improve the health of the community by reaching out to vulnerable populations.

Community Health Nurses:

  • Kids- promote immunizations and follow up on medical issues through connections with the schools (ECE, WSK8, JCMS, MHS). Provides medical services to children in the CPS group home through weekly clinics.
  • Adults- STD and outbreak investigations, provide medical assistance to WS Corrections inmates through weekly clinics.
  • Seniors- wellness checks as needed and helping with transitions when patients are discharged from the hospital.

Community Health Representatives:
Provide wellness checks and transportation assistance to medical appointments and dialysis.

Social Worker:
Provides comprehensive services to people of all ages experiencing psycho-social stressors. Receives referrals from physicians, community members, the schools, and other departments in the community. Patients may also refer themselves for Social Work services.

Dietitian:
Works with patients to improve health through making lifestyle changes including diet and exercise. Receives referrals from the Diabetes Programs, physicians and self-referral. Collaborates with other programs such as OSU Ext, ECE, Diabetes Prevention to provide educational opportunities to enhance the health of the community.

Commonly Asked Questions

What days are transportation available? Tuesday and Thursday for medical appointments 8:00am-4:30pm. Request for transportation must be made 1 week in advance. Please give appointment slip or referral to CHR’s.

What services do the Community Health Nurses provide at the clinic? Pregnancy tests, STD screening and treatment, birth control counseling and depo-provera injections, immunizations- for catch up in between Well Child Checks. Must be at least 4 months old to receive immunizations at Community Health. Please call ahead to make sure a nurse is present when you arrive.

How do I make an appointment with the dietitian? Call the Community Health Services front desk and ask for the dietitian.
